My paperwork that I had to fill out was the EXACT same information that I filled out online as its requested to do paperwork beforehand. Also, I was given the wrong paperwork for the wrong vaccine. PA didn't know what I was there for and was about to give me a COVID test instead of vaccine. I hope I am able to get my 2nd appt scheduled and the front desk does call in a timely manner as it was told to me. I don't have much confidence so I will be calling. Everything leading up to the actual administering of the vaccine, but not including that, was NOT done efficiently, friendly or accurately. Definitely room for improvement.
