Visit Info - Here is a list of things you’ll need to bring to your visit: 1) a valid form of identification, 2) an insurance card, 3) a method of payment, 4) a list of known allergies and medications, and 5) a mask for face covering.

COVID-19 Testing - We are required to complete a medical evaluation prior to testing. A provider will confirm your medical history, perform a physical exam and obtain samples for lab tests, which may include a nasal swab.

Was not satisfied with the nurse practioner who saw me. Had to go look up my symptoms and then told me she saw no evidence that I had that. I had lichen planus before so I know the symptoms and she said she couldn’t see them. Had no personality at all. Very disappointed I went there

When you're in pain you just want to be seen so you can go home and relax. I was in and out of there probably in under 45 minutes! The doctor even came out to the waiting room to talk to me which is almost unheard of! The nurses and doctors and x-ray technician were fabulous!

It was horrible, that doctor was so ignorant and the looks she gave, she didn’t listen to a word we said!!! I will never go there again!!!!!! The nurse kept apologizing for the ignorant doctor and I felt sorry for her having to be there with the doctor and feeling the need to apologize for her behavior!!!!! We have insurance and not welfare, but I felt like it was a waste of money and time, the 5hrs spent in a little room the dr saw us maybe less than 4 minutes total!!!!!!!!
